Many patients find dental insurance plans with their varying co-pays, confusing coverage terms, and restrictive care limitations quite a nuisance. Luckily, our knowledgeable staff can make handling your insurance claims and financial obligations a breeze.
As a courtesy to you, our front office staff will be happy to prepare all of the necessary forms for this important benefit. This will reduce your immediate, out-of-pocket expenditures. Our staff will provide your insurance company with necessary documents and X-rays at no extra charge.
However, we remind you that your policy is an agreement between you and your insurance company, not between your insurance company and our office. We can make no guarantee of any estimated coverage, but we'll do our best to see that you receive your maximum benefits. Your co-payment is due at the time service is rendered. Any balance not paid by your insurance will be your responsibility. Your prompt remittance is appreciated. Again, if you require special arrangements, this must be discussed prior to dental treatment.
To determine what your expected coverage will be, we can submit a pre-treatment authorization form. Your insurer will generally send a detailed response within 4-6 weeks.
Cosmetic Dentistry and Insurance
Cosmetic procedures are generally not covered by most dental insurance policies; however, partial benefits are often available if the teeth are broken or have previous dental fillings, bonding, or crowns.

Scheduling
Appointments are reserved specifically for you, and we work very hard to stay on time by allocating plenty of time for the procedure scheduled, so we ask you to be on time, and notify us if you are going to be delayed.
Our office requires 24-hour notification if an appointment is to be missed as we make every effort to accommodate same-day appointments for emergency patients. If, for any reason you cannot keep a scheduled appointment, we ask that you please notify our office as far in advance as possible.

Initial Visit
Our philosophy is to spend as much time as necessary learning about every new patient's oral health, prior to making treatment decisions. Your initial appointment involves a consultation with the doctor to get to know you and listen to what your needs and wants are. A complete oral evaluation will be performed which includes:
- a thorough tooth exam
- periodontal (gum) exam
- temporomandibular joint (TMJ), bite, and head/neck evaluations
- and any necessary X-rays
Dental "cleanings" (prophylaxis) are not always done at this visit as your periodontal status needs to be assessed to determine what type of "cleaning" is required. After this is done, your diagnosis will be explained and treatment options will be presented.
Please assist us by providing the following information at the time of your consultation:
- A list of medications and prescriptions you are presently taking - i.e. heart medications, aspirin, anticoagulant therapy, etc.
- A list of over-the-counter supplements you are presently taking - i.e. vitamins, herbs, minerals, amino acids, supplements, etc.
- A list of medical conditions that exist - i.e. diabetes, high blood pressure, artificial heart valves, etc.
- Please bring your dental insurance card (if applicable)
- Driver's License
IMPORTANT: All patients under 18 must be accompanied by a parent or guardian at the consultation and subsequent visits.
